In most of our systems we have dedicated node(s) feeding an INET system that reaches out to multiple locations in a given city/town. The people doing the broadcast have a modulator and a drop location that they can plug into and send it back to our headend. We take that signal into a switch and when the switch see's video from the broadcast it over rides a channel for the customers to view.
Without knowing specifics of your systems setup, it's hard to point you in the best direction.
