PTL Test Equipment out of Jupiter Beach, Fl.  I don't have the # handy, but if you can't find it pm me and I'll send it to you later.   I just had mine done about 4 months ago....if you have a current calibration it will cost $90.  If your cal date is passed a year it's an additional $300.
