there's a few other things that can cause what appears to be a mid band hump...
these 2 in particular
330 gear in the tap run...i've dug these up when they were buried in the ground...your levels come in low on the high end and you equalize the crap out of the low-end.
multi-media taps...the little metal strip that passes rf/ac when you remove the face plate. sometimes these dont completely release when you put your tap back in, causing the high end to lose a few extra db. this causes the sweep response to appear to have a hump in the mid-band when in reality it is high-end roll off.
either way, its the same problem. calculate your loss on low, mid and high end. extra high-end loss can be caused by many things, but the fix is to isolate the problem span, replace tap or tdr line, etc.
