are you using sc upc connectors via the 3m hot melt kit 3560? have seen this before. unplug the connector from the patch panel if using a blue coupler upc to upc try using a green coupler apc to apc. makes no sense at all but i have seen it 3 or four times and still scratch my head. most troubling thing is the first connector looks good under a fiber scope. and worked fine for a few months. WTF????
