LESSONS LEARNED
The following are details of incidents that highlight the importance for
technicians to follow ALL Ladder Handling/climbing policies.
Do not over-reach while on working on a
ladder.
while attempting to test a tap housing
with his FVD. The technician lost his
balance and grabbed an energized strand,
resulting in an electric shock.
Remember to tie-off your body belt to the
strand or pole and cut drops at the house
first. The technician was working from
ladder placed on a strand without tying off
He improperly cut the drop at the
strand first and the sudden release of
tension "sling-shotted" the tech off the
ladder from 25 feet high. He fell to the
ground and suffered a broken neck,
resulting in permanent paralysis from the neck down
Special Attention:
Always tie-off your body belt to the
strand or pole when working from a
ladder.
Always position ladders properly and
cut drops at the house first.
Always maintain balance and avoid
over-reaching when working from a
ladder.
