This depends on who you will be doing work for also, most MSO already have a set payout for each of things you asked about. Some can be good some can be bad…Also they have a catch all payout code for harder stuff, but you have to get approval before it can be billed. When I did work for TWC I walked out each property with the construction supervisor because each job is different. Found out what really needed to be done and what they would be willing to pay. I should suggest you do the same...
