Hopefully you have copies of all the work orders. Then file locally where the work was performed. Just tell the court clerk what you want to do because they can get you the right paperwork but they can't tell you how to fill it out.
Make sure you file against all third party beneficiaries of your work product, as this way you can file liens on the plant itself, the home owners, the prime, and if your being pimped out the sub as well. Now prior to filing walk into the office of the cable co. or if your being pimped go to the primes office first and hand them copies of what you are filing and let them know that you really don't want to go this way but that you have a family to feed, and that they will eat no matter what. You will get paid. and if you are being pimped you may even cut out the middle man.
