Seriously,
Google DOT Physical and you can print the whole thing to take to the doctor. This includes the card he signs and you carry with your A or B license. The doctor may not have the form so print it or get one from the DOT folks.
Shop around for a price. If you have a hospitalization plan you may be able to charge the cost of the test as an annual physical. Otherwise it could be a few hundred for something no doctor would pretend to be a competent exam.
