Sounds like he was using a ez hauler by cabledads description. They are a good machine, but are only rated for 2500 lbs. That is about the equivalent of a class five 40' wood pole. If he was setting a 60 then they were over the recommended use.
Once again it looks as if a worker is killed by some ass trying to save a buck. Quite sure during the investigation, the company will say "we told them what that machine was rated for, not our fault".
It is possible a family is left without a provider, and it is time to tell people again, this could happen to you. Do not rely on the company to care for your loved ones. Do not sign off on safety meetings unless you fully understand what they have handed out. If you are aware of unsafe practices happening, keep a diary and let your family know were it is, in case you become the tragedy.
