This is not a mishap. This is a should've-never-been. Guys like this most of the time if they're a rookie I can't blame - it's the guys that didn't train them right, but there is a lot of common sense that should come into play here. But, as I always tell myself and others, if it was common sense, everyone would have it.
This guy should not be in the field, and this should be reported to his manager, backcharged for the job, and given remedial training. Keep in mind I said should, not like it'll actually happen.
