Your signals look good, But if the CMTS (Cable Modem Termination System or Headend) is set to other levels i.e. +5 to -5 that could be part. Lose or corroded connectors, water in the coax or tap. I have found that CM's work best without AMPs. Also be sure coax is not connected to say a surge protector, and also the power cord was NOT left bundled and twist tied together, as this will change the impiedance. Your drop should be rg6, 59 will work if it is new (haven't seen any new 59 latley). Check for a ground to power, static can build and create noise. Are those levels constant? and are ALL splitters 1000mhz and NO home made connectors, check for braid touching the center conductor. Make sure your modem is not near a cordless phone, refrigerator, microwave, unshielded speaker, HAM or CB radio or scanner.
Im sure most of these you have checked, but when it goes down look at the log(while it's off), IF the log states "Initialize Baseline Privacy".."Skipped" then it is without a doubt a provisioning problem at the office. Also ask the office what the Up time and the flaps are. High flaps are power adjustments within a given time, mostly caused by a bad connector or filter.
