Maybe a tight pullout? Use breakaway to prevent the fiber from sucking back into the casing. Or its a commscope problem. Just because its costly fiber optics they make mistakes too. Weather might cause the wire to expand or contract, but i think that would only affect the jacket the fiber carries in not the glass itself.
