Try taking the 3x4 out of the path and run the troubled outlet straight to the dish.
I have had a similar situation and it was the customer's power outlet causing the problem. It worked fine for me without the 3x4, when I added the 3x4 it had signal problems similar to what you are having. To get the customer going until a electrician came out, I hooked up a second dish to power the troubled reciever independantly and this worked.
