Adaptis Amplifier termonal and SMPS burned


When I powered a node (SA 6940 + 8 Nos. Adaptis Amplifiers) with 60V alpha power supply, the input terminal and SMPS of the amplifier just before the end of line one has burned out. The voltage at last amp was 34V, and the burned one was 37V.

As per the spec sheet, the cut off voltage for adaptis is 35V. My questions are
1. How come the amp burn at 37V?
2. The design EOL voltage was 40V. Does the P3-500 cable resistance increase with aging?
