Could also be overdriving the laser, they're easy to send into clipping if the RF levels are too hot... if it's OK, try:
Scope the fiber connectors; replace RX; TX; try spare fiber... test MER after each
Also, when testing MER, hit test equipment with same RF level at each location, using pads to equal level at lowest level location
