Look for voltage feeding back from the computer via the cat-5 jumper or USB cable. A poorly wired outlet (reversed polarity) or a computer power supply going bad can bleed voltage to it's case which is effectlvely the neutral side of the two wire system. Use an outlet tester for the polarity and ground test. Use a volt meter with black to your drop sheath (which should be a good positive ground) and red (+) to the computer case, USB sheath (there will be DC voltage on the USB pins) and the cat-5 pins. If you have any more then 2-3 volts A/C it will eventually ruin the modem.
