Subject: Direct Activations Installer Payment Process


We have agreed to pay installers for installs made on behalf of Direct Activation that were completed between July 16, 2004 and August 6, 2004 . Payments will be made at our standard installation rates of $100, $125, and $165. Any claims for activations outside of the above dates should be directed back to Direct Activations.

Payments will be made only to those installers who have an installation agreement directly with Direct Activations. Subcontractors will need to work with their respective installation contractors for any payment issues.

Process:
Installers should be directed to call Retail Services @ 1/800-323-1994 for instructions. Retail Services will log the request and e-mail the attached letter, W9, claim form and our agreement to the installer.

The installer should return the completed packet along with their Direct Activation installation agreement to Kitty Alexander. Once they have forwarded the information they can submit their claim via e-mail to (-REDACTED-) . All payment claims must be submitted by September 15th 2004.
