You pose an interesting question. In my travels, it has been my experience that while you may be talking about major markets, the highest penetration of active homes vs. homes passed are in the smaller systems. The ones that dont show up on NASDAQ.
As to why, it usually is predicated on the fact that the systems I speak of are located in regions that may have only one or two local stations, or the systems are located in a valley between mountains and not able to receive good off air.
I have seen some smaller systems in Pennsylvania that have as much as 90% penetration. That was several years ago and with the advent of satellite those numbers may have dropped, but as I recall, there was a lot of loyalty to the local cable companies and dishes were frowned upon.
