Is each upstream channel dedicated to a specific US port on the CMTS? For example, if the output of the receiver is split to 4 US ports, any bad connection, pad, splitter port or CMTS port itself will cause this kind of issue. This is of course after checking the port configurations. Check your losses from the RX output to each US port using a CW, making sure that all US ports see the same RF level.
